How the HealthViber Living in Control™ Program Works

They follow national DSMES standards and the Tiny Habits® behavioral method so that you receive both clinical expertise and practical, real-life coaching in every session. What to Expect in Each Session

This session centers on the ways that diabetes can affect your body. You will talk about glucose, blood sugar levels, and how fasting numbers help show how your body responds. We will also go over how your daily habits can change your numbers significantly and review key health markers, such as your blood test results and body mass index, so that you understand where you stand and where you can improve.

The HealthViber Wellness Diabetes Prevention Program
The HealthViber Wellness Diabetes Prevention Program uses proven methods to teach you blood sugar management. Delivered through the HealthViber Wellness network of certified nurse facilitators, this program brings clinical expertise directly into your community, your neighborhood, your church, your barbershop, your community center. Our diabetes prevention program curriculum follows guidance from the Centers for Disease Control and Prevention and the American Diabetes Association and aligns with the National DPP standards for diabetes education. This program also supports diabetes self-management training and follows recognized DSMES standards. While this is a community-based diabetes education program where you get support from others, it does not replace the advice of your doctor.
You will be guided by experts who are trained in diabetes care, receive a clear plan to follow, and be part of a community that is there for you. This program is for adults who have type 2 diabetes, people with prediabetes, or anyone who wants to prevent these health issues. This diabetes self-management education program is perfect for people who have tried other plans in the past but found them too hard to follow. Our approach reflects a Tiny Habits® diabetes program model that helps small changes stick.
